Medicare Diabetic Footwear
ShoeLab helps eligible Medicare patients navigate the process of obtaining therapeutic shoes and inserts for diabetes.
Medicare may cover therapeutic shoes and inserts when specific medical and documentation requirements are met. Coverage is subject to Medicare guidelines and individual eligibility.
Do I Qualify?
To qualify for Medicare’s therapeutic footwear benefit, you must meet ALL of the following requirements:
You Have Diabetes
You Have at Least One Qualifying Foot Condition
Your certifying physician must document at least one of the following conditions:
Previous amputation of the foot or part of the foot
History of a foot ulcer
History of pre-ulcerative calluses
Peripheral neuropathy with evidence of callus formation
Foot deformity
Poor circulation/peripheral vascular disease
Your Diabetes is Being Managed Under a Comprehensive Care Plan
Your certifying physician must document that you are being treated for diabetes under a comprehensive plan of care and that you need therapeutic shoes.
Your Physician Must Complete the Required Certification
The physician managing your diabetes must complete and sign the required certification documenting your diabetes, qualifying food condition, comprehensive diabetes care, and need for therapeutic footwear.
You Must Receive an In-Person Footwear Evaluation
Before your specific shoes or inserts are selected, ShoeLab must conduct and document an in-person evaluation to determine the appropriate footwear for your needs.
Your Footwear Must Be Properly Fitted
When your shoes and inserts are delivered, ShoeLab must conduct and document an objective assessment of the fit.
What Medicare May Cover
For eligible patients, Medicare limits therapeutic footwear coverage to the following per calendar year:
1 pair of depth shoes + 3 pairs of inserts
Certain shoe modifications may be covered in place of inserts when Medicare requirements are met.
Coverage is subject to Medicare’s requirements and applicable deductibles, coinsurance, and plan rules.
